Lyramed User Agreement

Effective Date: February 7, 2026

This User Agreement ("Agreement") is a legal agreement between you and [Lyramed Legal Entity Name] ("Lyramed," "we," "our," or "us") governing your access to and use of the Lyramed platform, including our web application, AI chat assistant, document analysis features, profile management, and appointment-related features (collectively, the "Service").

By creating an account, clicking "accept," or using the Service, you agree to this Agreement.

1. Important Medical Notice

  1. The Service provides AI-assisted health information and administrative support only.
  2. The Service is not a substitute for licensed medical care, diagnosis, or treatment.
  3. Using the Service does not create a doctor-patient relationship with Lyramed.
  4. Do not delay or disregard professional medical advice because of Service output.
  5. If you think you may have a medical emergency, call 911 (or your local emergency number) immediately.

2. Eligibility and Account Use

  1. You must be at least 18 years old to create an account, or have legal authorization to act for a minor/dependent.
  2. You are responsible for keeping your login credentials secure.
  3. You are responsible for all activity under your account.
  4. You agree to provide accurate and current information.

3. What the Service Does

The Service may include:

  1. AI-based symptom and health-information guidance.
  2. Educational explanations of labs, imaging, or uploaded documents.
  3. Health profile and medication/allergy/vitals record tools.
  4. Appointment or diagnostic test booking/cancellation workflows.

Some outputs are generated by AI and may be incomplete or incorrect. You must use independent judgment and seek professional care when needed.

4. Appointment and Scheduling Terms

  1. Appointment availability shown in the Service may be preliminary and subject to confirmation.
  2. A booking request is not final until confirmed by the relevant provider or system workflow.
  3. You are responsible for reviewing confirmation details, attendance, and cancellation deadlines.
  4. Lyramed is not responsible for third-party provider delays, reschedules, or cancellations.

5. Document Uploads and Analysis

  1. You may upload medical records, lab reports, images, and related health documents.
  2. You represent that you have the legal right and necessary consent to upload all submitted data.
  3. You grant Lyramed a limited license to store, process, analyze, and display uploaded content solely to operate and improve the Service as described in our Privacy Notice.
  4. You are responsible for verifying AI-generated document summaries and interpretations with a qualified clinician.

6. Privacy, Data Use, and Consent

  1. Your use of the Service is also governed by our Privacy Notice: [Insert Privacy Notice URL].
  2. By using the Service, you consent to the collection, use, storage, and processing of personal and health-related information as described in the Privacy Notice.
  3. You acknowledge that electronic transmission and storage involve inherent security risks; while we apply safeguards, no system is completely risk-free.

7. Prohibited Conduct

You agree not to:

  1. Use the Service for unlawful, abusive, or fraudulent purposes.
  2. Submit false, misleading, or impersonated identity/health information.
  3. Attempt to reverse engineer, disrupt, probe, or bypass Service security.
  4. Upload malware, harmful code, or content you do not have rights to use.
  5. Use the Service to create medical misinformation or unsafe clinical instructions.

8. AI and Service Limitations

  1. The Service may be unavailable, interrupted, or contain errors.
  2. AI outputs may vary over time and between sessions.
  3. Lyramed does not guarantee that outputs are accurate, complete, or fit for any medical or clinical decision without professional review.

9. No Warranty

The Service is provided "as is" without warranties of any kind, express or implied, to the fullest extent permitted by applicable law.
